Digital Signature For Model Release (Type Full Name) *

Please read my Studio Policies below: Proofs are not printed and do not come with the session. What is the session fee? The session fee pays for time, artistic vision, digital editing and your private on-line gallery. This fee is non-refundable. In order to hold your appointment, the session fee must be received in advance in order to hold your appointment date. Payment can be sent via Pay Pal (photosbylsb@aol.com), credit card (Mastercard or Visa) or personal check. Any session rescheduling or cancellation must be made at least 48 hours before the portrait session or your session fee is forfeited. Lori Barrett Photography, Inc. does have a minimum print order of $250. Images will be available approximately 14 days after your portrait session. You may share this gallery with family and friends. This gallery is live for 7 days before it expires. There is a $25 re-hosting fee for expired galleries. Approximately 20 - 30 images will be available online for viewing. Once you have reviewed the gallery, an ordering session appointment may be scheduled. Payment for portraits is due in full at the time the order is placed. All orders placed during an ordering session will be ready 2 weeks after full payment is received. Purchased images can be picked up at the studio or mailed for an additional fee. When images are picked up at the studio, the photographer will ask you to inspect your images upon delivery and your signature is required stating they meet your approval. All sales are final upon delivery. If images are delivered by mail, you must contact the photographer within 3 days of receipt with any concerns or your right to reprints is waived and the sale is final. Images from your session are stored for 3 months from your portrait session date. After that time, all files may be purged and may not be available for printing or purchase at a later date. Lori Barrett Photography, Inc. retains the copyright and ownership of all images of the photo session. With a signed model release, Lori Barrett Photography, Inc. reserves the right to reproduce images for portfolio, studio display, gallery display, website, self-promotion, photo contests or any other professional development purpose. In order to control the quality of the work produced, only the photographer can crop, modify, transfer or print any image produced at the photo session. Clients are not allowed to distribute, print, download or reproduce any images produced by the photographer for any purpose, nor can the client authorize any reproductions of the images by others for any purpose. If digital files are purchased, clients will be issued an authorization to print release. You will also be given web sized images that are slightly watermarked for your protection for on-line use. Do to the nature of digitial negatives, once purchased, all sales are final. No other camera or equipment other than the photographer’s is allowed at the photo session. Pricing for services is based on the Standard Price List in effect on the date of this agreement. Prices are guaranteed for a period of 30 days following the date of the portrait session.
